How to Choose the Right Hospital Bed for Home: Semi Fowler vs Full Fowler vs Electric

You have decided to rent a hospital bed. You open the catalogue and see twelve options. Semi fowler, full fowler, recliner, one-function electric, two-function electric, three-function electric, three-function ICU, five-function ICU. You have no idea where to begin.

Before looking at any bed specifications, answer this: how frequently does the patient need to change position during the day?

  • Rarely, mostly staying in one comfortable position: a manual bed is sufficient
  • Moderately, a few adjustments per day: a manual bed with good crank mechanism works
  • Frequently, multiple times per day or night: an electric bed is worth the cost

The crank mechanism on a manual bed requires a caregiver to physically adjust it. If the caregiver is elderly, physically limited, or the patient needs repositioning at night, the electric motor more than justifies the additional rental cost.

Semi Fowler Bed

Elevates the backrest to approximately 30 to 45 degrees. Has a knee break that elevates the lower section independently. Manual crank operation. This is the most commonly rented hospital bed in India and handles the majority of home care situations.

May be considered when the written care plan calls for manual backrest adjustment. Suitability depends on the prescribed positioning, mobility, transfer method, mattress compatibility and caregiver support.

Full Fowler Bed

Elevates the backrest to 90 degrees, meaning the patient can sit fully upright in bed. Both backrest and knee section are independently adjustable. Manual crank. Used when the patient needs to be in a fully seated position, which a semi fowler cannot achieve.

May be considered when the treating team has specified the required upright positioning and the confirmed model provides the needed adjustment range.

Recliner Bed

Not a traditional hospital bed layout. Designed for patients who spend extended hours in a seated-to-reclined position rather than lying flat. The entire surface reclines as one unit. Useful for cardiac and respiratory patients who cannot lie flat.

1-Function Electric Bed

Single motor controls backrest elevation electrically. Knee section manual. Removes the physical effort of cranking for the most frequently adjusted part of the bed.

May be considered when frequent backrest adjustment is prescribed or practical for the care plan. Confirm the exact function list and the patient’s ability to use the controls safely.

2-Function Electric Bed

Two motors: electric backrest and electric knee section. Both adjust with a button. No manual cranking for either primary adjustment. A good balance of functionality and cost for most long-term home care patients.

May be considered when the care plan requires the model’s listed adjustment functions. Stroke, neurological and long-term care needs should be assessed individually.

3-Function Electric Bed

Adds electric height adjustment to the two-function. The entire bed can be raised or lowered at the press of a button. This is significant for caregivers: raising the bed to working height for care tasks, then lowering it for the patient to exit safely, eliminates back strain over a long care period.

May be considered when height adjustment is required for the individual transfer and care plan. It can support caregiver working posture, but does not by itself prevent falls or replace safe handling.

3-Function ICU Bed and 5-Function Electric ICU Bed

Advanced positioning functions vary by manufacturer and model. Trendelenburg, reverse Trendelenburg and chair-position functions should be confirmed in the product specification and used only when included in the written clinical care plan.
